- Information collected and disclosed about you requires your consent
- Your medical record is a confidential document and is only available to authorised staff members.
Your medical information will only be disclosed under certain specific circumstances, such as:
- For the diagnosis and treatment of your problem including communicating with practice staff, specialists and other healthcare providers involved in your care.
- For the billing of professional fees.
Requests for your medical information should be made in writing to the practice.
However, should you wish for someone else to have access to your information, eg obtain test results, medical information etc. on your behalf, a signed authority form must be completed. Please ask at reception for the forms and to explain the process. You can also view our privacy policy here.
