However, should you wish for someone else to have access to your information, eg obtain test results, medical information etc. on your behalf, a signed authority form must be completed. Please ask at reception for the forms and to explain the process. You can also view our privacy policy here.

Your personal details

It is very important that we have up to date contact details for you. Please inform us of any changes in your address, phone number, emergency contact details and any changes involving your Medicare or Centrelink entitlements.

Consent for Participation in Research

From time to time we will participate in research projects which may require personal health detail of patients. If identified patient health information is required, we will ask patients for written consent.
